  1. All teachers and associates have Elementary Citizenship Forms.
  2. Student completes the form.
  3. Parents are contacted.
  4. The person issuing the form makes one copy prior to sending it home for parent signature.  It is important that the teacher issuing the form collects it the next day.  Copies go to the principal and classroom teacher.
  5. The principal will keep an accumulative notebook of the returned forms.
  6. Who contacts the parents when a form is issued?
  7. Typically the classroom teacher or the teacher issuing the form will make this contact.  However, exceptions could occur due to the severity/frequency of the incident.  On these occasions an administrator will contact the parents.

    Making this contact in a timely manner is helpful to the developmental aspect of this process.
